  • Proposer un h\u00e9bergement d\u2019une journ\u00e9e \u00e0 3 mois pour offrir un temps de r\u00e9pit aux aidants<\/li>\n\n\n\n
  • Proposer un accueil de jour pour offrir un temps de r\u00e9pit aux aidants<\/li>\n\n\n\n
  • Poursuivre les soins pratiqu\u00e9s au domicile ou \u00e0 l\u2019h\u00f4pital avec l\u2019aide des professionnels de sant\u00e9 (Osmose, m\u00e9decins de ville, \u00e9quipes hospitali\u00e8res mobiles\u2026)<\/li>\n\n\n\n
  • Proposer un bilan complet m\u00e9dical pour \u00e9viter aux patients et aux aidants des transferts et des rendez-vous<\/li>\n\n\n\n
  • Mettre en place des ateliers th\u00e9rapeutiques compl\u00e9mentaires : art-th\u00e9rapie, musicoth\u00e9rapie, socio-esth\u00e9tique\u2026<\/li>\n\n\n\n
  • Proposer un accompagnement spirituel<\/li>\n<\/ul>\n\n\n\n

    Soutenir les aidants<\/h3>\n\n\n\n
      \n
    • Proposer des ateliers de sensibilisation aux aidants: s\u00e9curit\u00e9, manutention des personnes, pr\u00e9vention des chutes\u2026<\/li>\n\n\n\n
    • Proposer une r\u00e9\u00e9valuation de la prise en charge au domicile avec l\u2019aide de professionnels: m\u00e9decins, ergoth\u00e9rapeute, assistants sociaux\u2026<\/li>\n<\/ul>\n\n\n\n

      Soutenir les familles (patient, aidants, proches)<\/h3>\n\n\n\n
        \n
      • Mettre en place des temps de parole et de rencontre au patient et\/ou aux aidants pour les soutenir psychologiquement<\/li>\n<\/ul>\n\n\n\n

        Publics cible et modalit\u00e9s d\u2019accueil<\/h2>\n\n\n\n
          \n
        • Toute personne \u00e2g\u00e9e de plus de 60 ans n\u00e9cessitant une aide \u00e0 domicile\n
            \n
          • Accueil d\u2019une journ\u00e9e \u00e0 3 mois par an, en continu ou en fractionn\u00e9 selon le souhait du patient<\/li>\n<\/ul>\n<\/li>\n\n\n\n
          • Les patients chroniques \u00e0 domicile dont l\u2019\u00e9tat se d\u00e9grade progressivement vers une d\u00e9pendance accrue\n

            • Aucune prescription m\u00e9dicale n\u2019est exig\u00e9e<\/li>\n<\/ul>\n<\/li>\n\n\n\n
            • Les patients apr\u00e8s l\u2019\u00e9pisode aigu d\u2019une maladie et les patients n\u00e9cessitant des soins palliatifs\n
